Waivers can be accessed below. For the GHSFL and the FCS Insurance Verification you can submit digitally thru the link or you can download and return in hardcopy.
Georgia High School Fencing League Waiver
Click the link to Digitally Sign the GHSFL waiver. You can also click the download button to download and print a hardcopy.
NOTE: The GHSFL requires (2) Parent Signatures. The digital form can be saved and continued, but it is easier to sign when both parents are together.
FCS Insurance Verification Form
Click the link to Digitally Complete the insurance verification. You can also click the button to download hardcopy instead.
NOTE: You will be required to upload a photo of your insurance card.
Preparticipation Athlete Physical Form
Preparticipation physicals are required by the GHSFL before the first tournament. They are different than full physicals. There are a couple of options to meet the requirement:
- If your fencer already had one completed for a Summer Camp or another sport in the past 6 months we can accept that.
- If your fencer has had a check up at their doctor's office in the past year, they will usually complete the form for you without another visit.
- You can schedule an appointment with their doctor. They are familiar with this process and usually have separate appointment types for these physicals.
- They can be completed at any CVS or Walgreens minute clinics usually for around $40.
